Who are Raleigh Healthcare?
Raleigh Healthcare PCN brings together four local GP practices; Churchfields Medical Practice, Greenfields Medical Practice, Lime Tree Surgery, and JRB Healthcare to provide seamless, patient-centered care. As a Primary Care Network, our role is to enhance the services offered by individual practices through collaborative working, ensuring patients benefit from a wider range of healthcare professionals and services close to home. We focus on the needs of our community, working with partner organisations to deliver high-quality, accessible care.
