A social prescriber is here to support your overall well-being by helping you access community services and resources that can improve your physical, emotional, and social health. They focus on addressing non-medical challenges that may impact your health, such as loneliness, financial difficulties, or lifestyle changes. Social prescribers work with you to understand your needs and connect you with practical solutions in your local community. From accessing social groups to finding help with housing or money matters, they aim to empower you to make positive changes and feel more in control of your health and well-being.
